Current preview behavior
The consultation form is in preview mode by default. Inputs remain in the current page’s memory, are not sent to ADP and are cleared by navigating away or reloading. No client records, payment, appointment reservation or marketing enrollment is created. Do not enter medical information.
Age-aware contact
For athletes ages 13–17, a parent/legal guardian supplies their own contact details. Athletes ages 18–19 use adult permissions. The public form does not request a minor’s full birth date, school address, medical history or private contact information. An authorized adult contact is not automatically entitled to another adult’s private records.
Assets and hosting
Google Fonts supplies typography with system fallbacks. Images are hosted with the site and are AI-generated illustrations, not real ADP clients, staff or facilities. No advertising pixels, analytics trackers or browser storage have been added. The hosting provider may process ordinary technical access information under its own terms.
Before public operation
The actual operator must publish verified identity, contacts, inquiry processing and retention practices, processor disclosures and applicable rights. A developer must configure a secure inquiry endpoint, server validation and actual staff follow-up before enabling live submissions. If live delivery is enabled, the published policy must accurately describe that behavior.
